- Locate and then hit the claim form from the menu bar options.
- Enter all the required details such as name, address, city, zip code, telephone number, vehicle ID Number, vehicle model, etc.
- Enter the Total Amount Reimbursed and Paid for Repairs in Dollars.
- Attach the required document if needed.
- Accept all the terms and conditions.
